Birth Friday, 16 March 1877 in Little Lever, Lancashire.

Baptism Saturday, 28 April 1877 in St. Matthew's Church, Little Lever. To Samuel & Dinah, by James Slade. Samuel's occupation stated as mechanic.
 

According to the 1881 census, Samuel is the fourth of five children born to Samuel and Dinah (née Jackman) living at 41 Nob End Row in Little Lever. His father is a millwright.

 

According to the 1891 census, Samuel is now the fourth of nine children, now living at 7 Alpine Terrace in Kearsley. His father is a fitter in the local foundry, whilst Samuel himself is a joiner's apprentice.

 

According to the 1901 census, five of the children, not including Samuel, are living with their parents at 1 Alpine Terrace. Their father is a millwright.

Marriage to Alice Simmons, on Wednesday, 3 July 1901 at St. John's the Evangelist Church, Farnsworth-under-Kearsley, Lancashire. Living at 1 Alpine Terrace, stated as being a moulder.

Sam and Alice Wolstenholme had two daughters together. Lily Agnes (b.5 November 1902) and Alice (b.19 February 1907).

His mother died in Little Lever, 31 May 1909.
According to the 1911 census, Samuel is a professional athlete, married to Alice, with two children, Lily and Alice. They live at 55 Muriel Road in Norwich.
His father died in Little Lever, 1 July 1914

According to the 1921 census, Samuel, now an iron moulder, still married, and with his two daughters and his brother, James, as a boarder, they live at 12 Darley Street in Farnworth.

According to the 1939 register, Samuel, a football scout, and Alice, are still married, living at 6 Preston Street in Bolton with their younger daughter, Alice.

"Sam Wolstenholme, the Blackburn Rovers, Everton, Croydon and Norwich City half-back, who 40 years ago made three appearances for England, has died in hospital aged 68." - The Evening Express, Saturday, 20 October 1945

The death previously given as March 1933 is wrong, that Samuel Wolstenholme was born in 1879.
Funeral Tuesday, 23 October 1945 at St. Michael's Church, Manchester Road, Great Lever.
  His wife, Alice Wolstenholme, died in early 1956.

Club(s) Began his football career with Darley Vale FC. Shortly followed by Farnworth FC and Farnworth Alliance FC. Time spent with Horwich FC before he left for Everton FC late in 1897. Blackburn Rovers FC signed him on 12 May 1904. Croydon Common FC signed him on 19 June 1908 as club captain and Norwich City FC in the first days of May 1909. Signed with Chester FC, becoming their captain in the 1913-14 season.
