According to the 1881
census, the seven-year old Frederick R. is the third of four children,
including Frank, to Ralph and Élise (née Kershaw), a French-born British
subject. They all live on the Weston Road in
Aston, Shardlow. His father is a farmer of 65 acres, employing two men and
two boys. |

According to the 1891
census, the family of six are all still living in Weston Road and his
father is still a farmer. Frederick is a signal engineer. They now
have one servant. |

morning, 14 June 1910 at The Balcony, Ocean Avenue in
Skegness, Lincolnshire, he had been
battling illness for a couple of years and died of Phthisis Pulmonalis
Exhaustion (Tuberculosis). |

"DEATH OF FRED FORMAN
"Fred Forman, brother of Frank, and at one
time outside right, and then outside left for Nottingham Forest, died
yesterday morning at Skegness, after an illness extending over a couple of
years. Fred Forman was 37 years of age, and after playing with
Aston-on-Trent, Beeston and Derby County, as an amateur, he joined Forest
in 1893, and in 1899 got his triple cap for England as outside left
against Ireland, Wales, and Scotland. Forman, in his day, was a brilliant
forward, and undoubtedly owed much to the early coaching of John Goodall.
The deceased was unmarried."

"Funeral of Mr. F. R.
Forman at West Bridgford.
"The funeral
took place at West Bridgford yesterday afternoon of the late Mr. Frederick
Ralph Forman, who some years ago occupied a prominent place in the realms
of Association football. Throughout the last part of his career he was a
member of the Nottingham Forest Club, and so excellent was his form at
outside right that he obtained International distinctions. Not so long
after he retired from the game Mr. Forman's healtrh broke down, and he
passed away at Skegness on Tuesday last. Many friends and those who had
known the deceased, who was of a most amiable disposition, assembled in
West Bridgford Parish Church for the first portion of the burial service,
which was takjen with music. The chief mourners were the members of the
late Mr. Forman's family. The officiating clergy were the Rev. James S.
Holden, rector of Aston-on-Trent, the Rev. P. Fleming, and the Rev. R. D.
Weller, curate at West Bridgford. As the coffin was borne into the church
the Marche Funebre was played, and the Dead March from 'Saul' as the
procession moved to the graveside, and the beautiful hymn 'On the
Resurrection Morning' was sung." - Nottingham Daily Express,
